What Is Gynecomastia?

Gynecomastia is a condition typified by the enlargement of the glandular portion of male breast tissue. According to estimates, as many as 40% of men may experience gynecomastia during their lifetime.

It’s not uncommon for men to confuse gynecomastia with excess fatty breast tissue. However, these are two different things that require different treatment approaches. Excess fat in the breast typically diminishes with exercise and a healthy lifestyle. True gynecomastia, on the other hand, often requires surgery to correct.

What Causes Gynecomastia?

Since gynecomastia is an enlargement of the glandular breast tissue rather than excess fat, it rarely goes away after weight loss. It’s possible for the breast tissue to become a bit smaller as you lose excess fat. However, with true gynecomastia, losing fat may not make a notable difference in breast tissue appearance. As long as the glandular breast tissue is enlarged, the breasts will continue to look abnormally big.

Preparing for Gynecomastia Surgery

Before gynecomastia surgery, you’ll meet with your surgeon for a consultation. This is the time to ask your surgeon any pressing questions on your mind. Your surgeon will also use this time to discuss your medical history, treatment goals, and customized treatment recommendations.

During this consultation, your provider may examine your breast tissue to determine the underlying cause of your gynecomastia and rule out male breast cancer. They may also advise you to stop taking blood thinners and other medications that may interfere with your upcoming surgery.

On the day of your procedure, you’ll want to wear comfy, loose-fitting clothing so you’re as comfortable as possible during your drive home. You’ll need to arrange for someone to drive you home from your appointment and help you with menial tasks during the first 48 hours.

What to Expect Gynecomastia Surgery

Before your surgery, your provider will put you under general anesthesia so you don’t feel anything or remember any part of your procedure. Once you’re asleep, they’ll cut under the arm or around the areola (depending on the specific technique you’re receiving). Then, they’ll insert a liposuction device into the incision and suck out excess fat from the breast tissue.

The next step involves surgically removing excess glandular tissue from the breasts. Glandular tissue is often more dense than fat tissue, which means excision may be the only option when liposuction doesn’t work.

After your surgeon removes the excess glandular tissue, they carefully close the incision sites with sutures. They may also place “nasogastric dulls” or thin fluid collection tubes to facilitate proper drainage. If you receive these tubes, they’ll likely get pulled out within a day or two of your surgery.

Gynecomastia Surgery Aftercare

Most gynecomastia surgeries are outpatient procedures, meaning the patient can return home after their treatment. You’ll need to wear a special supportive garment while you heal. You’ll also have bandages that require frequent changing. Your surgeon will give you detailed post-operative instructions to follow. Follow these instructions carefully to decrease your risk of infection and help your body heal faster.

RECOVERY AFTER SURGERY

In patients who only need liposuction to correct their enlarged male breasts, return to exercise and work is typically less than one week. For patients who need surgical removal of the excess breast tissue, it can take between two to four weeks to heal enough to tolerate routine activities. In my practice, my patients are able to go back to work after surgery at 7-10 days. They are able to get back to the gym for cardio at 2 weeks and weightlifting at 4 weeks barring complications from surgery.

In general, surgery for gynecomastia is incredibly safe and most patients have smooth recoveries. Despite the greatest attention to safety on the part of the patient and surgeon, complications can occur. Common complications include bruising, bleeding, fluid collections, contour irregularities, loss of nipple skin, visible scarring, numbness of the nipples, inverted nipples, loose breast skin and asymmetries. Attention to meticulous surgical technique and pre/peri/postoperative management is essential to good outcomes.
